Cornerstone conducts a structured intake and assessment process to identify each child’s functional needs, physical limitations, environmental supports, and family priorities.

Services are developed in collaboration with the parent or legal guardian and are based on medical documentation, functional assessment, and approved service authorization. An individualized support plan is established to address mobility, positioning, safety, and participation in daily routines. Plans include measurable goals, risk considerations, and defined caregiver responsibilities.
Services are reviewed and updated according to DMH requirements, care coordination guidance, and changes in the child’s condition or functioning.
